How to take the Wegovy pill: fasting, timing and doses

On waking. One tablet, a little water, and half an hour before breakfast.Tilly, the app's turtle, holding a glass of water in one hand and a single pill on her other palm, a small sun above her

The Wegovy pill is semaglutide, the medicine in the Wegovy and Ozempic pens, as a tablet you take once a day. It was approved in the US in December 2025 and in the UK in June 2026, in both for weight management.1,2 The tablet swaps the weekly shot for a daily habit, and the habit comes with a rule: on waking, on an empty stomach, a little water, then half an hour before anything else.3 Most of what goes wrong with the pill goes wrong in that half hour. Here is the rule, the reason for it, and the four steps of the first months.

The rule, as the label writes it

  • Take one tablet in the morning, on an empty stomach, with water only: up to 4 ounces, which is about 120 millilitres. Not with coffee, tea or juice.3
  • Swallow it whole. Do not split, crush, chew or dissolve it.3
  • Then wait at least 30 minutes before eating, drinking anything else or taking other tablets.3
  • One tablet a day, never two. If you miss a day, skip it and take the next one the following morning.3

The UK label says the same thing a little more precisely. An empty stomach means after at least eight hours without food, and the water is a sip, up to half a glass.4 A night’s sleep is those eight hours, which is why the rule lives in the morning.

Why a tablet needs a rule

Semaglutide is a peptide, a small protein, and the stomach is built to take proteins apart. A tablet of it has to get through the stomach wall before that happens, and only a small share of it does. For semaglutide tablets, the Rybelsus label puts the share that reaches the blood at roughly 0.4 to 1 percent, absorbed mostly in the stomach itself.5 The doses are set with that small share in mind, so it is enough. But it also means anything else in the stomach during the half hour, food, a big glass, coffee or another tablet, competes with the semaglutide and cuts the dose you actually get.

The Wegovy pill is the same molecule, taken by the same rule, for the same reason. The Rybelsus page on this site goes through the measurements behind the 4 ounces and the 30 minutes, if you want the detail.

The night before: the tablet and a small glass of water on the bedside table, so the morning needs no thinking.

Four steps, a month each

The pill starts low and climbs. The US label’s schedule is 1.5 milligrams a day for the first 30 days, 4 milligrams for the next 30, then 9, and from day 91 the 25 milligram dose you stay on.3 The UK label has the same four steps, each for at least a month, and says the dose can stay at the previous step if needed.4 The tablets come in exactly those four strengths.3

The low steps are there for your stomach, the way the pens start low. If a step is hard going, the US label says your prescriber can consider delaying the next increase, and if the 25 milligram dose is not tolerated, switching to the weekly 1.7 milligram injection.3 Staying longer on a dose you can live with is not falling behind. It is how many people reach the dose that works for them, and it is a decision your prescriber makes with you.

Two things about the numbers. They are not Rybelsus numbers: Rybelsus comes in 3, 7 and 14 milligrams, and even between the semaglutide tablets its own label covers, the same milligrams do not mean the same dose.5 Moving from one tablet to another is a prescriber’s plan, not a sum. And the Wegovy label advises against taking it together with other semaglutide products, pen or tablet.3

In Europe, the medicines agency’s committee gave a positive opinion on the tablets, in the same four strengths, in May 2026.6 Whether and when a pharmacy near you has them depends on your country.

The stomach in the first months

Nausea is the side effect most people meet first. In the 64-week tablet trial in the UK label, 47 in 100 people on the pill reported nausea at some point, against 19 in 100 on placebo. Vomiting was 31 against 6, constipation 20 against 10, and diarrhoea 18 against 9.4 The US label says the tablet’s common side effects were similar in kind and frequency to the pen’s.3

The shape is different from the pen’s, though. A weekly shot tends to have a hard day, the day after. A daily tablet keeps the level flatter, topped up every morning, so there is no day of the week to brace for. What there is instead is a new step every month for the first three months, and those first weeks on a new step are the ones to plan around.

What helps is what helps on the pens:

  • Small and plain. Half portions, eaten slowly, and stop a few bites before you feel full, because on these medicines fullness arrives late.
  • Protein first, fat last. Fat slows the stomach further, and a slow stomach is what makes you queasy. Eggs, yogurt and chicken sit better than anything fried.
  • Sips, not glasses. Through the day, not a pint at once.
  • Stay upright for twenty minutes after eating.

If you cannot keep fluids down, or there is pain in the upper belly that does not pass, call your prescriber rather than wait it out.

A morning built around it

The people who keep a morning rule for years do not rely on remembering it. They build a morning where the rule is the easy path.

  • The glass by the bed. The tablet and a small glass of water on the bedside table the night before. The dose is taken before your feet touch the floor, before the question of coffee comes up.
  • The half hour is the shower. Thirty minutes is a shower, getting dressed and making the bed. Set a timer for the first weeks if it helps.
  • Coffee after, not with. The coffee waits, and so do the juice, the smoothie and the toast.
  • Other tablets after the half hour. The label asks for the wait before other medicines taken by mouth, too.3 If you take a thyroid tablet or anything with a narrow safe range, ask your prescriber how to space them before you rearrange anything.
  • Make the first meal count. When the half hour is up, the appetite is often small. Start with protein: eggs, Greek yogurt, cottage cheese, a protein shake if food is too much.

Pill or pen

They are the same medicine and they behave the same way once inside: semaglutide stays in the body with a half-life of about a week, whichever way it goes in.5 The difference is the habit. The pen is one shot a week and a harder day after it. The pill is a rule every morning and a flatter week. Which one suits you is your prescriber’s call, and often a question of which habit you would rather keep.

Tilly knows the Wegovy pill as a daily medication with the morning rule built in: log the tablet and Today tells you when the half hour ends, “Water only until 7:40. Breakfast after that.” It never tells you what dose to take or when to step up. That is between you and the person who prescribed it, and the label they are reading is the same one this page is built on.

Questions

Can I take the Wegovy pill with coffee?

No. The label says water only, up to 4 ounces, which is about 120 millilitres, and then at least 30 minutes before any other drink or food. The coffee comes after the half hour.

Can I take the Wegovy pill at night?

The label ties it to the start of your day: in the morning, on an empty stomach, then half an hour before anything else. At night, after dinner, the stomach is not empty, and the UK label asks for at least eight hours without food first. If mornings are hard, ask your prescriber before you move it.

What is the starting dose of the Wegovy pill?

By the US label, 1.5 milligrams a day for the first 30 days, then 4 milligrams, then 9, and from day 91 the 25 milligram dose. Your prescriber sets the steps and can keep you on one for longer.

What if I miss a day?

The label says to skip the missed dose and take the next one the following day, never two tablets in one day. If you are unsure what to do, your pharmacist is a quick call.

Is the Wegovy pill the same as Rybelsus?

It is the same molecule, semaglutide, but a different medicine: different strengths and a different use, and the milligrams are not interchangeable. The Wegovy label also advises against taking it together with other semaglutide products.
